Overview
From birth to geriatrics, our clinic sees all patients. You can visit us for a wide variety of healthcare needs. From regular preventative care to managing chronic conditions or helping you and your family through illnesses, our primary care providers are here to help. We are also an Internal Medicine residency clinic.
If you do not have a primary care physician and have been recently discharged from the hospital, you have the option of scheduling your follow-up visits at our clinic.
Services at Cape Fear Valley Internal Medicine Continuity Clinic
General and preventive services to all patients age 18 and older
Care to Cape Fear Valley employees (who need to be seen ASAP) and cannot get in with their PCP
Chronic disease management of diabetes, hypercholesterolemia, hypertension, COPD, asthma, Transition Care Management (TCM)/hospital follow-up, etc.
Physicals, Medicare Wellness visits, preventive care and general/acute medical care
Osteopathic manipulative medicine (OMM)-comprehensive
Weight loss teaching/referral if applicable
