Coordination of Care
Coordination of Care nurses provide case management and collaboration of healthcare providers, specialists, nursing and support teams to provide comprehensive patient care. They perform screening for appropriateness of admission, insurance reviews, discharge planning. They monitor length of stay and use of hospital resources.
Candidates
- Three years of nursing experience and BSN preferred.
Employee Health
Employee Health Nurses administers compassionate care to workers in an occupational setting to promote the health and safety of CFVH employees. Services include preventative care such as immunizations, pre-employment examinations, and annual evaluations, monitoring environmental health hazards, and providing assessment, treatment and case management for employees who are injured during work or exposed to blood-borne pathogens.
Candidates
- Three years of nursing experience. Experience in public health, occupational or employee health preferred.
Infection Prevention & Control
Infection Prevention is a rapidly changing field in which nurses coordinate activities to prevent and investigate healthcare-associated infections, protect patients and staff by working with public health partners on the control of emerging and existing infectious diseases, develop education for staff and community members, and provide oversight on construction and renovation projects.
Candidates
- Two years of experience in nursing, public health, epidemiology or microbiology is preferred. New infection prevention nurses are provided additional training by UNC and APIC membership. Professional growth through academic advancement and specialty certification are encouraged.
Nursing Quality & Safety
Nursing Quality & Safety nurses are responsible for monitoring quality of care and services. They apply improvement and evidence-based methods to achieve successful outcomes. They synthesize and analyze data from various sources to identify opportunities and make recommendations for improvement. The assess risk for harm by using tools and methods such as PCA and FMEA and facilitate the development of action plans in response to survey or audit reports.

- Two years of nursing experience and BSN preferred.
