Other reasons include a burst (perforated) appendix, an abscess, or, less commonly, concern for a growth or tumor. There are two main surgical approaches. A laparoscopic appendectomy is performed through several small incisions using a camera and specialized instruments. A ruptured appendix can lead to peritonitis or sepsis and must be treated quickly. Special situations:
- Children: Symptoms can be less specific and may progress to perforation faster, so rapid evaluation is essential.

Preparing for an Appendectomy
Before surgery, your team will complete a preoperative evaluation. This may include blood tests, a pregnancy test when appropriate, imaging (ultrasound or CT), and a review of your medications and allergies. You will discuss the procedure, benefits, risks, and alternatives, and sign informed consent.
If your surgery is urgent but not emergent, you will be asked to fast (no food or drink) for a period before anesthesia, commonly starting at midnight or as directed. Tell your team about blood thinners, diabetes medications, and any supplements. Some medications may be adjusted or held the day of surgery. Avoid smoking or vaping, and remove jewelry and contact lenses.
At the hospital, you will check in, change into a gown, and meet your surgeon, anesthesiologist, and nurses. An intravenous (IV) line will be started for fluids, antibiotics, and medications. What Happens During an Appendectomy
Most appendectomies are performed under general anesthesia, so you are asleep and pain-free. Throughout the operation, your heart rate, blood pressure, oxygen level, and breathing are continuously monitored.
Laparoscopic appendectomy: The surgeon makes several small incisions and gently inflates the abdomen with carbon dioxide to improve visibility. A camera and instruments are inserted to separate the appendix from surrounding tissues and blood vessels. The appendix is placed in a protective bag and removed through one of the incisions. The area is irrigated, bleeding is controlled, and the small incisions are closed with sutures or skin adhesive. This minimally invasive approach to appendectomy surgery often allows patients to resume activities sooner.
Open appendectomy: A single incision is made in the lower right abdomen. The surgeon locates and removes the appendix, controls bleeding, and irrigates the area. This approach may be selected if the appendix has ruptured with significant infection, if there is extensive scarring, or if the laparoscopic approach is not ideal. In either case, the goal is safe and complete appendix removal.
If the appendix is perforated or there is an abscess, a temporary drain may be placed to remove infected fluid. For a large, walled-off abscess, an initial drainage procedure with antibiotics may be done before or instead of immediate surgery, with appendectomy performed later.
Risks, Complications, and Benefits of Appendectomy
All surgeries carry risks. Common risks include wound infection, bleeding, pain, and bruising. Less common risks include injury to nearby organs such as the small or large bowel, urinary issues, blood clots, hernia at an incision site, or anesthesia-related complications. After a perforated appendix, an intra-abdominal abscess can develop and may require antibiotics or drainage.
Benefits include definitive treatment of appendicitis, faster relief of symptoms, prevention of rupture, and a reduced risk of serious infection. Most patients return to normal activities quickly, especially after laparoscopic surgery.
Risk varies by approach and by individual factors. Laparoscopic surgery typically offers smaller incisions, lower wound infection rates, and quicker recovery for many patients, although it may take slightly longer to perform. Open surgery may be preferable with severe inflammation or dense scar tissue. Age, obesity, diabetes, smoking, immune suppression, and a ruptured appendix can increase the chance of complications.
