Emily Ghassemi, PharmD, BCACP, CDCES, CPP

Dr. Ghassemi is a board-certified ambulatory care pharmacist (BCACP) and certified diabetes care and education specialist (CDCES). She graduated with a PharmD and MS in Clinical Research and completed two years of residency specializing in ambulatory care pharmacy and academia. Dr. Ghassemi practices as a Clinical Pharmacist Practitioner in the family medicine continuity clinic. In this role, she collaborates with other members of the healthcare team to manage chronic medications, provide pharmacotherapy services, patient education, and assist with drug information and medication access resources.
