How To Refer A Patient

A written referral from physician or referral source is required for Well on Your Weigh. To avoid unnecessary testing, the primary provider should fax all relevant medical records, test results and X-rays prior to the appointment date.

Please provide the following when referring children for a diagnostic evaluation:

• Patient name and date of birth
• Probable diagnosis
• Insurance/referral information
• Physician clinical findings or symptoms which caused the patient to visit physician.
