Thank you for your interest in Cape Fear Valley Rehabilitation Center. We have been providing rehabilitation services to residents of southeastern North Carolina since 1976
We provide comprehensive adult and pediatric programs specializing in treatment for amputations, brain injuries, neurological disabilities, orthopedic injuries, spinal cord injuries, and stroke.
Inpatient Rehabilitation
To refer a patient to be screened for an inpatient rehabilitation admissions:

Please be prepared to provide the listed patient information:
- Patient name
- Patient’s current location
- Patient’s diagnosis
- Patient’s insurance and demographic information
- History and Physical
- Therapy evaluations
- Any pertinent operative notes
- Name and contact number of the Case Manager
Criteria for inpatient rehab referrals
The patient must require comprehensive interdisciplinary, medical rehabilitation to include the services of at least two therapies for a minimum of three hours of therapy per day, five days per week, 24 hour rehabilitation nursing care, and medical supervision.
The patient must have identifiable goals, and a reasonable expectation for functional improvement.
The patient must be medically able to participate.
The patient must have an established discharge disposition.
