What is myChart at Cape Fear Valley?

myChart at Cape Fear Valley offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With myChart at Cape Fear Valley, you can use the Internet to:

  • Request, schedule and cancel medical appointments
  • Communicate electronically and securely with your medical care team.
  • View your health summary from the MyChart electronic health record.
  • Read your providers' notes from your office visits
  • View test results.
  • Request prescription renewals.
  • Access trusted health information resources.
  • Pay your bill online


Is there a fee to use MyChart?

myChart at Cape Fear Valley is a free service offered to our patients.


How do I sign up?

At check-in, you can request to sign up during your next clinic visit or use the activation code found on your printed After Visit Summary to sign up at mychart.capefearvalley.com. If you were not issued an activation code, you may request one by navigating to https://mychart.capefearvalley.com/MyChart/signup and entering key demographic information:

  • First and Last Name
  • Address
  • DOB
  • Legal Sex
  • SSN
  • Email address
  • Phone Number

If a successful match occurs you will be directed to create your username and password. If you were unable to be matched successfully to your medical record, based off of the information that was entered, you will receive a message that your request has been sent. We will work to get you connected to your medical record and logged in quickly.

When can I see my test results in MyChart?

Most test results are automatically released to myChart at Cape Fear Valley within 48 hours of being resulted, with the exception of certain sensitive tests. It may take several days or weeks for some other tests to appear. Once available, you can discuss the results with your provider. Some sensitive results may be held up to 8 weeks before being automatically released in myChart to provide an opportunity for you to discuss the results with your provider. If test results are automatically released and you have not discussed your questions or concerns with your provider, please contact your provider. Please note that results prior to 5/3/2019 will not be available in your myChart at Cape Fear Valley account.


If I send a message to my doctor or nurse, when can I expect a reply?

You will receive an answer within 3 business days. Please note that myChart at Cape Fear Valley should not be used for urgent situations. Please contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.


Can I view a family member's health record in myChart?

Yes you can. This is called Proxy access and allows a parent (or guardian) to log into their personal MyChart account, and then connect to information regarding their family member. Complete a MyChart Proxy Access Request for Minor Form (0-12) and return it to one of our clinics or to medical records to request access to this convenient service.

If you are an adult that is requesting proxy access to another adult you will need to complete the MyChart Adult Proxy Access Form and return it to one of our clinics or to medical records for proxy access to be granted.


Can I ask questions regarding a family member from my MyChart account?

myChart at Cape Fear Valley offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.


How do I terminate proxy access?

Proxy Access can be terminated by completing the MyChart Proxy Access Termination Form at your next visit.


Can my teen (between the ages of 13-17) have access to MyChart at Cape Fear Valley?

Yes, myChart at Cape Fear Valley is happy to provide your 13 to 17-year-old child with his or her own account. An added benefit of your 13 to 17-year-old having his or her own myChart at Cape Fear Valley Account is that the teen will then have access to information like their medication list, a list of upcoming appointments, online appointment scheduling for return visits and even online appointment reminders. As a parent proxy account holder, under most circumstances, you will not have direct access to this kind of information in your Proxy Account for the teen once they turn 13 years old due to NC privacy law.

In order for you to gain proxy access for a teenager (aged 13-17), the parent(s) or legal guardian(s) must first complete the myChart at Cape Fear Valley MyChart Proxy Access Request Form and Authorization for Teenaged Minor form. Additionally, the teenager must sign a Proxy Access Authorization Form for Release of Information. Once these forms are completed they can be returned to the teen's provider clinic or medical records for an invitation to be sent to the proxy.


I forgot my password. What should I do?

You may click the "Forgot password" link on the sign-in page to reset your password online. If you need additional assistance please call our myChart at Cape Fear Valley Support Line at 866-860-2098.


Where can I update my personal information (e.g., home address, e-mail or change my password)?

Log into MyChart and navigate to the Profile icon in the upper right section of the screen, go to the Personal Information section and select the appropriate option.


What appointments can I schedule in MyChart at Cape Fear Valley?

You can schedule an appointment with a provider you have seen in the past 12 months in the clinic where your visit occurred who is participating in myChart at Cape Fear Valley appointment scheduling. This includes primary care visits such as adult annual exams, pediatric well-child annual exams and follows up visits. Additionally, if you need ongoing treatment for an existing condition that requires a return visit with your specialist, you may also schedule that visit in your MyChart at Cape Fear Valley account.


What is the difference between scheduling an appointment and requesting one?

Scheduling an appointment means you have secured a date and time with a provider in a clinic where that provider practices.

An appointment request is used when you are unable to schedule with that provider in myChart at Cape Fear Valley and you need assistance from our appointment team to secure a date and time with that provider. This is useful for new problem visits with a specialist you have never had a visit with.


Why do I not see my provider listed as a scheduling option?

Providers are available for scheduling in myChart at Cape Fear Valley if you have seen that provider in a clinic visit within the past 12 months. If your provider is not available to schedule with online or, you need to schedule a visit with a specialist for a new or undiagnosed problem.


Offers of Earlier Appointments

If patients already have a scheduled appointment with their primary care provider or specialist, they can elect to be added to a list in the event an earlier appointment becomes available.

When speaking to a scheduling representative on the phone or in person, patients will be asked if they would like to be added to the list. If an earlier appointment becomes available, patients will receive an email directing them to log in to their myChart at Cape Fear Valley account or a push notification if you have already downloaded the MyChart Mobile App. Once logged in, patients can see the details of the earlier appointment and elect to accept the earlier appointment offer or keep their existing appointment. Earlier appointments are offered on a first-come, first-serve basis. If a patient has not accepted the offer, it will expire the following morning at 7am.


How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.


What is your Privacy Policy?

myChart at Cape Fear Valley is owned and operated by MyChart and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyChart.


I was logged out of MyChart, what happened?

We aim to protect your privacy and security of your information. While logged into myChart at Cape Fear Valley, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.


Accessing myChart at Cape Fear Valley on your mobile device

With the myChart at Cape Fear Valley mobile app you can schedule appointments, view test results, medications, immunizations, proxies and appointments.

The myChart at Cape Fear Valley mobile application is currently available on the Apple® App StoreSM and Android Google Play store.

Instructions:

  1. Download MyChart to your device, Launch the MyChart application and select Cape Fear Valley as your healthcare provider.
  2. Log in using your MyChart username and password

What is Happy Together?

You can use your myChart at Cape Fear Valley account to view your health information from other organizations, where you and your family receive care, through Happy Together. That means, if you receive care at different organizations, you no longer have to log into multiple MyChart accounts as long as those organizations have also implemented Happy Together.

When you log into your myChart at Cape Fear Valley account, you will be able to see information from other healthcare organizations where you receive care. The information you'll be able to see includes:

  • Allergies
  • Care Team
  • Health Issues
  • Medications
  • Messages
  • Test Results
  • Visits

When your MyChart accounts are connected, you will be able to send messages to your healthcare provider at any of the locations where you receive care. It's convenient and easy to manage your healthcare needs from a single system.


How to Start Using Happy Together

It is up to you whether you would like to link accounts. If you have information from another healthcare organization, you will see a screen asking if you would like to connect your accounts. If you change your mind about linking your accounts, you can remove the link any time by going to Settings > Manage My Accounts and uncheck the Connect Account button.
