Notice and Frequently Asked Questions about a patient data breach
This notice is provided by Cumberland County EMS and LifeLink, in regards to a data breach incident affecting records of a number of EMS and LifeLink patients. Advanced Data Processing, Inc., manages patient billing for both agencies. On Oct. 1, 2012, Advanced Data Processing learned one of its employee improperly accessed and disclosed certain patient account information in connection with a scheme to file false federal tax returns. The accessed information included a name, date of birth, Social Security number and record identifier. No medical information was breached.
What happened to the employee?
The employee was subsequently arrested and terminated as an Advanced Data Processing employee, no longer having access to company systems. The company has thoroughly investigated the matter.
What should I do if I suspect my data has been compromised?
If you suspect your personal information is being misused, you should contact local law enforcement and file a police report. If you suspect a tax return has been illegally filed using your information, you should contact your local IRS Service Center or call the IRS at (800) 908-4490. We advise you to remain vigilant and monitor your credit reports periodically. The Fair Credit Reporting Act requires each nationwide consumer reporting agency to provide you with a free credit report, at your request, once every 12 months.
